Online enrollment application for AHCCCS Health Insurance, KidsCare, Children’s Rehabilitative Services, Nutrition Assistance (food stamps/SNAP), and Temporary Assistance for Needy Families (TANF) case assistance.
Application can be completed online, mailed/faxed, in person at a local DES office, or with a Community Assistor.

- Documents required often include: proof of citizenship or alien registration cards, social security cards for all family members, proof of relationships of family members (birth certificates work), contact information of landlord or neighbor, written statement verifying all individuals residing together made by a non-relative that is signed, dated, and includes contact information for person signing (address and phone), proof of ALL income for the last 60 days, recent bank statement, proof of rent/mortgage, proof of utility bills, proof of child care expenses, etc.
