Eligibility – women who are at nutritional risk, pregnant, breastfeeding, and/or postpartum and infants/children age 5 and below with income below 185% of federal poverty guidelines.
Some WIC appointments can be completed in person at your home. Call your local WIC office for more information.

Required Documents: Birth Certificate, Social Security Card, Photo ID
- Documents Required often include: identification for adults in home, identification for children (can be shot record, birth certificate, crib card, or hospital ID), proof of income for the last 30 days, and proof of address (mail or bill, driver’s license, rent receipt, etc).
